Problems with La Sportiva Nepal Extreme

Hi,
I'm writing because I have problems with La Sportiva Nepal Extreme mountaineering boots. Specifically on the lower outer part of the foot very soon severe pains appear. Actually it happens even if I have the boots on before I even start walking. In the boot you can feel a swollen part there. How can this be resolved?
So far I've worn the boots about 20 hours.
On the attached image I've marked the area where the pain occurs.
For hiking the boots I don't tighten too much in the lower part, only in the upper. I lace the summer boots La Sportiva Trango Trek Micro Evo GTX the same way and no such problems.
